GENERAL INFORMATION

The target number of attendees is 240.

You can register your delegates either by group registration or individual registration. Please indicate on the registration form how many delegates, including company personnel, you want to register for the course. After receipt of your registration you will receive a confirmation. Please be informed that the commitment deadline is on 15th March 2009. After this date, registration will be handled upon request and we cannot guarantee full availability. The number of registrants can be changed without any additional costs until March 1st 2009.

COURSE DINNER

This year the course dinner will be organized in collaboration with the International Myeloma Foundation. Dinner will be served in the Fairmont restaurant on Friday 15th May.

The costs per attending person are ˆ 115,00 (including VAT and tourist tax)

Robert A. Kyle Lifetime Achievement Award

Preceding the dinner, the Robert A. Kyle Lifetime Achievement Award will be awarded to Jean-Luc Harousseau. The IMF's Robert A. Kyle Lifetime Achievement Award was established to honor an individual whose lifetime body of work furthers the ultimate goal of finding a cure for myeloma, a cancer of the bone marrow. The accolade is named after Dr. Robert A. Kyle, noted physician and founder of the Myeloma and Related Diseases Research Group at the Mayo Clinic in Rochester, Minnesota. Dr. Kyle himself was the first recipient of the award in 2003. For more information about IMF please visit their website at www.myeloma.org.

HOTEL ACCOMMODATION

Attendees will be accommodated in Fairmont Monte Carlo hotel.

Room rate is inclusive of breakfast buffet, VAT and service charges:

Single Fairmont room: ˆ 200 incl. breakfast (excluding tax)
Double Fairmont room: ˆ 235 incl. breakfast (excluding tax)

All guests will be responsible for payment of their own personal incidentals (e.g. minibar, telephone).
Upon check-in the hotel will ask for a credit card as guarantee.

FLIGHT TRANSPORTATION
With regard to flight transportation, you will need to make your own arrangements. If you need assistance with flight arrangements, we will be happy to assist you at a handling fee of ˆ 35 per booking.

GROUND TRANSPORTATION
From Monaco Airport, taxis are available to take you directly to the Fairmont Monte Carlo Hotel. The drive will take up to 45 minutes. If you need assistance with on-site transportation we will be happy to assist you. We will arrange a chauffeur who will be waiting for you on arrival and take you together with other attendance’s (waiting time will be in a reasonable timeframe) to the Fairmont Monte Carlo Hotel. The rate will be ˆ 120,- (retour, excluding ˆ 15 handling fee). We will invoice you directly.

PAYMENT

Total payment for individuals or groups should be received within two weeks after submitting the registration, but no later than 1st May 2009. We will send you an invoice for the amount of the allocated places and accommodation within two weeks after receiving your registration. This will include a administration fee of ˆ 25 per booking.
